Ambulatory Surgery Centers

The Date of Last Survey represents the most recent date of a Medicare certification and/or State licensure survey. It excludes complaint investigations, which are recorded separately. Other surgery facilities exist in New Jersey as private physician practices, which do not require State licensure and may or may not be certified for the Medicare program.
